​​Before booking your appointment, please be aware that we do not participate with medical insurance plans (including Medicaid, Medicare, or TRICARE). We currently accept VSP vision plan only and do not accept other vision insurance providers.
 

If you are self-pay, please indicate "self-pay" in the notes section of the appointment request or select "self-pay: no insurance" from the insurance drop-down list and indicate "none" as your member number and "self" as the policy holder.

For VSP patients: please include the primary member's name, date of birth, and last 4 of their Social Security number or unique VSP ID # in the notes section of the appointment request form.
